How to collect a liability waiver on a sign-up form

To collect a liability waiver, add a required consent checkbox with your waiver text to the form. Each person must agree before they can sign up, and the acknowledgment is recorded on their response.

Steps

  1. 1 Add the waiver Ask the builder to add a required waiver, for example "require a liability waiver checkbox", and paste or describe your waiver wording.
  2. 2 Make it required The checkbox is required, so a person can't complete the sign-up without acknowledging it.
  3. 3 Keep the record Each response records that the waiver was accepted, so you have the acknowledgment alongside the rest of their sign-up.
