How to collect a liability waiver on a sign-up form

To collect a liability waiver, add a required consent checkbox with your waiver text to the form. Each person must agree before they can sign up, and the acknowledgment is recorded on their response.
Steps
1Add the waiverAsk the builder to add a required waiver, for example "require a liability waiver checkbox", and paste or describe your waiver wording.
2Make it requiredThe checkbox is required, so a person can't complete the sign-up without acknowledging it.
3Keep the recordEach response records that the waiver was accepted, so you have the acknowledgment alongside the rest of their sign-up.
